All Men Are Not Created Equal Featured in Cosmetic Surgery Times

Contrary to the U.S. Declaration of Independence, not all men are created equal or identify themselves equally.

Dr. SteinbrechThese contradictions have propelled Douglas Steinbrech, M.D., FACS, a  plastic and reconstructive surgeon with locations in Manhattan and Beverly Hills, to create different categories for men to identify with on his website

“There are many different types of men,” Dr. Steinbrech tellsCosmetic Surgery Times. “Initially, the website was to educate men,” he says. “However, the website has also really increased the volume of men in my practice.”

Case in point, upon opening his practice, in Manhattan, 12 years ago, Dr. Steinbrech’s clientele comprised about 80% women and 20% men. Today, the practice treats roughly 80% men and 20% women.

Three years ago, to help drive male business, Dr. Steinbrech created four categories of males on his website: the model (or in Los Angeles, the actor), the athletic dad, the bodybuilder and the CEO/boardroom executive. Each category also lists several recommended procedures.

The Model

“The most popular procedure for the model is abdominal sculpting,” Dr. Steinbrech says. “Through hidden incisions, I sculpt the abs and achieve a dramatic reduction of fat to the flanks of the love handles.”

The second most popular procedure is a male model “makeover” that creates a chiseled Superhero jawline via fillers or permanent implants. Of the two, “we usually use fillers to inject into the cheekbone and accentuate the jawline and the chin,” Dr. Steinbrech explains. “I always tell my model patients that every guy has a better-looking ‘twin’ working in Hollywood. That counterpart has slightly higher cheekbones, a little wider jawline and a more projecting chin. By tweaking, we want to create this better-looking twin.”

Dr. Steinbrech says “twin” is the ideal word to convey to patients. “We don’t want the patient to look like someone else,” he explains. “We still want the patient to look like himself and appear natural; in other words, the best-looking version.”

The Althletic Dad

The athletic dad also wants sculpted abs, perhaps along with a stronger chest and some submental liposuction or Kybella (Allergan) to erase a double chin.

The “Daddy Do-Over” is Dr. Steinbrech’s version of the Mommy Makeover that includes liposuction and high-def liposcultping of the abs, love handles and neck. “These patients are now focusing on their jobs and families, so they have sort of let go the body they had in their 20s,” Dr. Steinbrech says.

The Bodybuilder

The third male category, the bodybuilder, entails intense sculpting. “I call my set of principals ‘BodyBanking®,’ which is somewhat similar to bodybuilding but with fat,” Dr. Steinbrech notes. “With my patent-pending technique, I suck out the fat from the flanks, the lower chest and the abs, then I reinject that fat into the upper chest for a bigger chest, into the shoulders, the biceps, the triceps, the calves and even into the six-pack.”

According to Dr. Steinbrech, BodyBanking® prevents the fat from returning to the areas where it was removed, adds bulk to the newly transferred-fat areas, and keeps fat from popping up into new areas like intra-abdominal visceral fat.

Silicone implants for the chest, calves, forearms, abdomen, shoulders, biceps and triceps are also popular in this category.

The Executive

The executive type of male is seeking a cleaner neckline and fresher eyes, which often means an upper and lower blepharoplasty and/or a neck lift. “This patient wants to look better in the boardroom and be able to compete with the younger guys,” Dr. Steinbrech says.

In June, the website added a fifth male category: “The Biggest Winner,” a play on the popular TV reality show “The Biggest Loser,” which targets men who have gone through massive weight loss, mostly as a result of gastric bypass, with lingering excess skin that is addressed with any or all of the various body part lifts.

“These patients require circumferential body lifts,” says Dr. Steinbrech, who has developed a swimsuit lift, which conceals incisions lower, beneath the swimsuit line. “Currently, a lot of physicians make the same incision then they would for a woman—high upon the thigh and wrap around as if the man would be wearing a women’s one-piece bathing suit.”

Of the five categories, the male model represents about 30% of Dr. Steinbrech’s patients, followed by the athletic dad (20%), “The Biggest Winner” (20%), the bodybuilder (15%) and the executive (15%).

Dr. Steinbrech encourages other cosmetic practices to understand their own local male patient types, based on practice location. “For example, if you practice in South Carolina, there may not be a lot of actors or models,” he says.

Stratifying patients also helps to create an understanding of realistic aesthetic outcomes and allows people to identify with specific procedures.

Mirror, mirror on the wall, who’s the fairest of them all? Step aside, Snow White; men are paying closer attention to their reflections and investing in their looks with a newfound openness to cosmetic surgery.

Over the past 20 years, men seeking nips and tucks have shot up more than 325 percent, according to the American Society for Aesthetic Plastic Surgery. In order of popularity, men are lining up for nose jobs (rhinoplasty), eyelid surgery (blepharoplasty), breast reduction (gynecomastia), liposuction, and face-lifts, reports the American Society of Plastic Surgeons, and now account for more than 10 percent of plastic surgery patients.

While women have long felt the social implications of a world obsessed with youth, experts say that men are now feeling more of that pinch. “The reality is that the job market is fierce, and many of my patients come in to look and feel younger to boost their self-confidence,” says New York City plastic surgeon Douglas S. Steinbrech, MD, who specializes in male cosmetic surgery. “Guys want to stay competitive in the board room and feel and look good to clients and colleagues,” he says.

Is a chiseled jawline the new version of a shiny red midlife-crisis convertible? In addition to CEO and boardroom executive types, Steinbrech sees a lot of athletic men staving off the so-called “dad bod” with a “daddy-do-over” that tightens sagging faces and sucks away love handles and beer bellies that have slowly crept on over a couple of decades. Steinbrech even performs pectoral and butt implants on some male patients who want to recapture the jock look of college days gone by.

Whatever you may fancy, looking good doesn’t come cheap. Nose jobs, liposuction, and male breast reduction cost anywhere from $5K to $8K each on average, and face-lifts can run about $12,000. Eyelid surgery, which is approximately $5,000, is often an add-on to other anti-aging procedures for a completely refreshed look, since the eye area is often the first and most noticeable area to age.

Millennial men are also heading in to the cosmetic surgeon’s office, much earlier than generations before them, who often waited until retirement years to address advanced aging issues. They’re much more open and comfortable with the idea of plastic surgery, says Steinbrech. This age group tends to opt for more affordable injectable fillers and “Bro-tox,” which are often priced from only a couple hundred to a few hundred dollars and yet still significantly refresh the face.

Part of the draw is that cosmetic surgery — when performed by a board-certified and experienced doctor — is having far more natural results on men these days. Fears that you’ll end up looking like an aging D-list celebrity in a tabloid headline are diminishing. This month, Kybella, the recently FDA-approved injectable that dissolves neck fat, has been running a television commercial that stars the impressive before-and-after shots of a man looking to make his genetic double chin history.

Meanwhile, at cosmetic surgery community, interest in nonsurgical hair-loss treatments have skyrocketed among male readers, who are most concerned with taking action against their receding hairlines. Consumers visited the site to research and view before-and-after photos twice as much this year as compared with last year, show site metrics.

Follicular Unit Extraction — or FUE — remains the gold standard for both hair transplantation and hair restoration, says Austin, Texas, plastic surgeon Jennifer L. Walden, MD. SmartGraft is a new device that more quickly harvests hair grafts and places them in balding areas, so hair follicles have a better chance of surviving for fuller results. Either procedure starts at about $8,000 to $10,000 and can quickly rack up much higher, depending on the size of the treatment area and the time it takes to do the procedure.

Younger men aren’t leaving those hairlines to chance; Walden says she sees more men with premature thinning who want to intercept the process before it gets too visually noticeable. “Patients are more apt to call or consult for hair-loss treatments today and not feel embarrassed — cultural shifts have occurred, and cosmetic procedures are just more accepted and understood by the mainstream public,” says Walden.

Alex, a 30-something who works in financial services in New York City, recently had the popular FUE hair transplant. As someone who is sharply groomed and who works out regularly, he saw it as an extension of how he likes to present and take care of himself, both in his personal and professional life. “It’s vanity. I didn’t like the thinning and recession of my front hairline, so I handled it,” he tells Yahoo Beauty.

He paid $11,000 for the procedure and worked from home while recovering for a few days afterward, though he says the raw redness of his scalp took a couple of weeks to subside. And he learned that cosmetic surgery isn’t a magic wand and that expectations must be managed. “It is definitely an improvement, and I’m glad I did it — having said that, I expected there to be fuller coverage of the places where the hair follicles were plugged in,” he says. Would he consider other procedures? “If I felt Botox or a neck-lift were necessary at some future point, I’d consider — and maybe even more, if technology improves,” says Alex.

With male demand for cosmetic procedures only gaining, we think it’s a safe bet that cosmetic technology will do exactly that.

Bye Bye, Man Boobs

All summer long. Scorching hot days. At the beach. At the pool. Even on vacation. No matter where he went or what he was doing, my cousin refused to take his shirt off. Pretty strange for a guy, huh? Well, not as strange as you just might think…


Gynecomastia. That’s what my cousin suffered from, and while it definitely made him feel like an outcast, the truth was he was far from it. Also known as “man boobs,” Gynecomastia is the condition of enlarged male breast tissue, and believe it or not, it affects an estimated 40 to 60 percent of the male population — leaving most men feeling embarrassed, emotionally distressed, and with low self-esteem. Although my cousin felt alone every time he would hide his body under a baggy shirt, there’s unfortunately plenty of guys out there doing just the same, feeling ashamed and uncomfortable in their own skin — pretty much living within their own personal prison.

Understanding Gynecomastia:

Here’s the basics on what you need to know — Gynecomastia is excess localized chest fat, excess glandular tissue development, and/or even a combination of both. It can be present in either one or both of the breasts, but it doesn’t have to be there to stay forever.

In severe cases of gynecomastia, the weight of excess breast tissue may cause the breasts to sag and stretch the areola (the dark skin surrounding the nipple). In these cases the position and size of the areola can be surgically improved and excess skin may be reduced.

Gynecomastia is a condition of over-developed or enlarged breasts in men that can occur at any age. The condition can be the result of hormonal changes, heredity, obesity or the use of certain drugs.
